Feedback / Complaint Form
Positive feedback is welcome! Please submit your feedback below.
Students and employees who have a nonacademic issue or concern should first attempt to resolve the matter directly by speaking to the staff member involved. Should this approach fail, then they should submit written feedback using the form on this page. All pertinent information (date, a brief statement of the issue, etc.) should be included in the form so the issue can be resolved. Once received, the form will be forwarded to the appropriate college administrator for review. The person submitting the form will be contacted if further information is needed and also to report a resolution of the issue. The complaint resolution process for nonacademic matters, like housing or finance, can be found in Policy 4008 (PDF).
Students with an academic issue should first attempt to resolve the matter directly with the instructor. If that fails, students should contact the academic department for information on how to proceed.
The grade dispute procedure can be found in Policy 3011 (PDF) and procedures for allegations of academic dishonesty are found in Policy 3010 (PDF).
Distance Education students outside of Arizona, after completion of the institution’s internal complaint process, may appeal SARA related complaints to the Arizona Community College Coordinating Council (AC4). Additional information and instructions are found at the AC4 website.

The Arizona State Authorization Reciprocity Agreement (SARA) Council has jurisdiction over AZ SARA approved institutions regarding non-instructional complaints for distance education students. Upon completion of the institution’s and AC4’s complaint process, a student may register a complaint with the Arizona SARA Council. Visit the AZ SARA Complaint Process website for more information: .
In addition to submitting complaints to AZ SARA, Distance Education students who are taking distance education classes from outside of Arizona, may also submit a complaint to the state consumer protection agencies (website) listed in this document. However, please note that SARA member states will direct a submitted complaint back to AZ SARA as the home state of the institution.
